Job summary

Ideal Option is seeking a Case Manager in Dona Ana County, New Mexico to provide outreach and intensive case management for individuals suspected of low-level offenses. The role involves field-based engagement, collaboration with housing providers, and connecting clients with essential services and resources.

Applicants should have an undergraduate degree in social services or behavioral health, experience with harm reduction, and familiarity with community resources.

Qualifications

• Undergraduate degree in social services or behavioral health.
• Experience with CRM and EHR systems.
• Certified Recovery Mentor or Peer Support Specialist certification.
• Experience serving populations in harm reduction contexts.
• Experience with street-based outreach and criminal justice system liaison.

Responsibilities

• Provide Outreach and Intensive Case Management for assigned clients.
• Engage participants on the street and at social service facilities to establish relationships and offer services.
• Assess participants for chemical dependency severity and housing status; determine needs for medical/mental health services.
• Assist participants in accessing funding programs (SSI, ABD, VA).
• Assist participants in finding housing and maintaining occupancy.
• Advocate for participants within the criminal justice system including court appearances.
