Job summary

Ideal Option in Las Cruces, New Mexico is seeking a Case Manager to provide engagement and intensive case management services for individuals affected by substance use disorders. You will work on the front lines, conducting outreach, coordinating housing and medical services, and collaborating with partners to support participants through the criminal justice system.

Qualifications

• Experience serving populations impacted by extreme poverty and behavioral health conditions from a harm reduction model of care.
• Experience advocating for participants within the criminal justice system including court appearances and written communication.
• Street outreach experience providing services and resources to the populations we serve.

Responsibilities

• Provide Outreach and Intensive Case Management services for assigned clients.
• Engage participants on the street and at social service provider facilities to establish a working relationship and offer services.
• Assess participants for severity of chemical dependency and housing status and determine needs for other services.
• Assist participants in gaining access to funding programs (e.g., SSI, ABD, VA).
• Assist participants in finding housing and maintaining occupancy.
• Provide advocacy and support for participants within the criminal justice system including court appearances and written communication.
• Advocate for the participant with a wide variety of other service providers.
