Job summary

Adaptive Home Health is seeking a Certified Occupational Therapy Assistant (COTA) to provide skilled home health visits across Detroit-area regions. The role is field-based with a full-time schedule, Monday–Friday and occasional weekends as needed, supporting patients to stay safe and independent in their homes.

You will work under the OT plan of care, perform ADLs/IADLs, and coordinate with the care team to ensure quality outcomes.

Qualifications

• Active Michigan COTA license (in good standing).
• Strong clinical judgment and ability to work independently in the field within COTA scope.
• Strong communication and thorough documentation.

Responsibilities

• Deliver skilled COTA visits in the home under the supervising OT’s plan of care.
• Execute functional treatment plans: ADLs/IADLs, safety/fall prevention, energy conservation, UE function, adaptive equipment training.
• Track objective functional progress and communicate updates/ concerns to the supervising OT.
• Coordinate with nursing and the broader care team to address condition changes and safety concerns.
• Document promptly and thoroughly—defensible and efficient.
