Job summary

Adaptive Home Health, LLC is seeking a Certified Occupational Therapy Assistant (COTA) to perform field-based home health visits in the Detroit area under the supervising OT’s plan of care. You’ll deliver skilled COTA visits and implement ADL/IADL-focused treatment plans, while coordinating with the care team to ensure patient safety.

The role emphasizes on-site documentation, strong clinical judgment, and reliable travel across an assigned territory.

Qualifications

• Active Michigan COTA license in good standing.
• Strong clinical judgment and ability to work independently in the field (within COTA scope and under OT supervision).
• Strong communication and thorough documentation.
• Current driver's license and comfort with daily travel across assigned territory.

Responsibilities

• Deliver skilled COTA visits in the home under the supervising OT’s plan of care.
• Execute functional treatment plans: ADLs/IADLs, safety/fall prevention, energy conservation, UE function, adaptive equipment training.
• Track objective functional progress and communicate updates/concerns to the supervising OT.
• Coordinate with nursing and the broader care team: coordinate changes in condition and safety concerns.
• Document promptly and thoroughly — defensible, efficient, and consistent.

• Schedule: Full-time Monday–Friday + occasional weekends (as needed)

• Location: Field-based

The COTA role is the front line of that mission: you deliver high-quality functional visits in the home under the OT plan of care, helping patients stay safe and independent.

We build technology to better support our field clinicians faster and more supported. Ultimately, our clinicians are the customers of our engineering team. Our charting product is designed to reduce documentation burden and keep visits moving, and our scheduling product (launching soon) will make it easier to coordinate visits, reduce last-minute changes, and protect clinician time.
