Job summary

Adaptive Home Health is seeking a Certified Occupational Therapy Assistant (COTA) to perform field-based home visits across Texas. This full-time role operates Monday–Friday with a field-based location and compensation structured around productivity points.

You will deliver functional COTA visits, implement ADLs/IADLs and safety-focused plans under OT supervision, and collaborate with nursing and care teams. Travel across the assigned territory is required.

Qualifications

• Active Texas COTA license in good standing.
• Strong clinical judgment and ability to work independently in the field under OT supervision.
• Clear communication and thorough documentation.
• Current driver's license and willingness to travel daily across the assigned territory.

Responsibilities

• Deliver skilled COTA visits in the home under the supervising OT's plan of care.
• Execute functional treatment plans: ADLs/IADLs, safety/fall prevention, energy conservation, UE function, adaptive equipment training.
• Track objective functional progress and communicate updates/concerns to the supervising OT.
• Coordinate with nursing and broader care team; escalate changes in condition or safety concerns.
