Job summary

Adaptive Home Health is hiring a Certified Occupational Therapy Assistant (COTA) to deliver skilled visits in patients' homes across Detroit and surrounding areas. Under the supervising OT, you will implement the plan of care, support ADLs/IADLs, and promote safety, independence, and energy conservation.

We back you with experienced clinical and operational support, a clear productivity model, and scheduling tools to coordinate visits.

Qualifications

• Active Michigan COTA license (in good standing).
• Strong clinical judgment and ability to work independently in the field (within COTA scope and under OT supervision).
• Strong communication and documentation skills.
• Current driver’s license and willingness to travel daily across assigned territory.

Responsibilities

• Deliver skilled COTA visits in the home under the supervising OT’s plan of care.
• Execute functional treatment plans: ADLs/IADLs, safety/fall prevention, energy conservation, UE function, adaptive equipment training.
• Track objective functional progress and communicate updates/concerns to the supervising OT.
• Coordinate with nursing and the broader care team: coordinate changes in condition and safety concerns.
• Document promptly and thoroughly — defensible, efficient, and consistent.
