Job summary

Adaptive Home Health is seeking a Certified Occupational Therapy Assistant (COTA) to provide skilled visits in patients' homes across the Denver area under OT supervision. This field-based role emphasizes high-quality, patient-centered care delivering ADLs, safety, energy conservation, and functional training.

You will document promptly in HCHB, track patient progress, and collaborate with nursing and care teams to coordinate care. A Denver COTA license and reliable daily travel are required.

Qualifications

• Active Denver COTA license in good standing.
• Strong clinical judgment and ability to work independently in the field under OT supervision.
• Current driver’s license and willingness to travel daily within territory.

Responsibilities

• Deliver skilled COTA visits in the home under supervising OT’s plan of care.
• Execute functional treatment plans: ADLs/IADLs, safety/fall prevention, energy conservation, UE function, adaptive equipment training.
• Track objective functional progress and communicate updates to the supervising OT.
• Coordinate with nursing and the broader care team.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
