Job summary

Albuquerque Health Care for the Homeless is seeking a Community Support Worker to deliver trauma-informed, client-centered care to unhoused individuals across Albuquerque. You will manage a caseload of 20–30 clients and meet them where they are, using company vehicles to remove barriers to housing and care.

Qualifications

• Bachelor’s in a human service field or equivalent experience.
• Experience applying trauma-informed and harm-reduction approaches.
• Ability to work with unhoused individuals in the community.

Responsibilities

• Assess individual needs of clients.
• Create individualized service plans with client input.
• Coordinate wrap-around services with AHCH programs.
• Refer clients to appropriate programs and agencies.
• Document client contacts in the EHR within required timelines.
