Job summary

Family Residences and Essential Enterprises, Inc. is seeking an HCBS Provider to deliver non-clinical rehabilitation services in community settings as part of the HCBS Programs.

You will assess client needs and develop measurable, attainable plans within an interdisciplinary team. You will maintain EMR documentation, support Core Clinicians with a ~20 client caseload, and travel to client homes as required while upholding the organization’s Code of Ethics and confidentiality policies.

Qualifications

• Bachelor’s degree in a mental health or social services field is required or preferred.
• Experience with non-clinical rehabilitation is a plus.
• Strong communication and organizational skills.
• Valid driver's license and willingness to travel within Nassau and Suffolk Counties.

Responsibilities

• Provides non-clinical, rehabilitative services
• Effectively communicates and supports the organization’s mission, vision, and values
• Assists Core Clinicians with a caseload of approximately 20 individuals
• Communicates observations regarding goal progress to Supervisor
• Ensures documentation meets quality and compliance thresholds
• Utilizes electronic medical record to complete required documentation
• Provides field-based non-clinical treatment in the community and individual’s home
• Adheres to the organization’s Code of Ethics
• Maintains confidentiality
• Maintains and contributes to client caseload including non-clinical treatment planning and progress notes
• Prevents, recognizes and reports abuse toward the people we support
• Completes all required trainings
• Performs other duties as assigned
