Job summary

OLV Human Services is seeking a part-time Community Provider to deliver direct services to children and families in the community, including CFTSS and HCBS programs. The role emphasizes person-centered planning, autonomy for individuals, and collaboration with Care Managers.

The position requires driving to community/home visits, attends trainings, and maintains accurate service documentation using EHR. A NYS Driver's License is required.

Qualifications

• High School education with 2 years' experience in Mental Health, Substance Abuse Disorder, or Foster Care OR Bachelor's Degree in Human Services.
• Valid NYS Driver's License.

Responsibilities

• Provide direct services in community or home settings.
• Attend paid new worker training and regulatory trainings.
• Document services and progress notes in EHR.
• Coordinate with Care Managers to develop service plans.
• Maintain relationships with youth and caregivers.
