Centene is seeking a field-based Community Resource Specialist to connect members with community resources and support the care management journey. The role requires residence in specified North Carolina counties and offers a schedule Monday through Friday, 8:00 AM to 5:00 PM, with occasional evenings and weekends.

You will assist members in navigating community and care resources, document resources, and support clinical teams with outreach, education, and referrals.

Formación

• Requires strong communication and customer service skills.
• Experience in community outreach or field-based roles is preferred.
• Ability to perform non-clinical health assessments and coordinate community resources.

Responsabilidades

• Assist members in connecting to community resources and care services.
• Coordinate and promote awareness of community outreach resources.
• Support member inquiries and educational materials per plan requirements.
• Document and maintain community resources to meet health plan policies.
• Support clinicians and social workers through outreach, education, and guidance.
• Conduct non-clinical health and environmental assessments for referrals.
• Perform telephonic or in-person outreach to reach hard-to-contact individuals and families.
• May visit homes or community organizations as part of outreach.
