Job summary

Centene is seeking a Field-Based Community Health Worker in North Carolina to connect members with community resources and support care teams. You will coordinate outreach and assist with home visits to ensure access to essential services.

The role requires a High School diploma or GED and 1–2 years of related experience, with typical hours Monday–Friday, 8:00 AM–5:00 PM and occasional evenings/weekends. Select NC counties apply.

Qualifications

• Requires a High School diploma or GED.
• Requires 1–2 years of related experience.
• Experience in field-based maternal and child health services, community outreach, and conducting home visits is preferred.

Responsibilities

• Provide support to connect members to known community and care resources in a cost-effective manner.
• Coordinate community outreach resources and promote awareness of care/services.
• Assist the clinical team with outreach, education, and member support.
• Conduct non-clinical health and safety assessments to refer to services and gather information for providers.
• Document and maintain all community resources per health plan policies.
• May conduct home visits and telephonic outreach to locate individuals and families in the community.
• Comply with all policies and standards.
