Centene is seeking a field-based Community Health Worker to connect members with community and care resources, supporting the care management journey. The role emphasizes outreach, education, and collaboration with nurses and social workers in North Carolina counties.

The position offers standard hours Mon-Fri 8:00 AM - 5:00 PM with occasional evenings and weekends, requires a high school diploma and 1–2 years related experience, and pays $17.84-$28.02 per hour plus benefits.

Qualificações

• Excellent communication and customer service skills.
• Experience in field-based maternal and child health services or community outreach a plus.
• Ability to conduct home visits and non-clinical assessments.
• Comfort working both independently and in a team.

Responsabilidades

• Provides support to connect members to known community and care resources in a cost-effective manner.
• Coordinates outreach resources and promotes awareness of care/services.
• Assists members with community/resource inquiries and opportunities.
• Documents and maintains community resources per policies and standards.
• Supports clinical team with outreach, education, and member support.
• Conducts non-clinical health assessments to refer to care/services.
• Performs telephonic and in-person outreach to locate hard-to-reach individuals.
• May visit homes or community organizations.
• Works with Social Determinants of Health (SDOH) barriers.
• Ensures compliance with policies and standards.
