Responsibilities

• Supports community connection activities and links members to community resources.
• Coordinates outreach resources and promotes care/services awareness.
• Assists the clinical team with outreach, education, and member support.
• Conducts non-clinical health assessments to refer members to appropriate services.
• Performs telephonic or in-person outreach to locate hard-to-reach individuals and families.
