Job summary

Companion Health Services LLC seeks a Community Liaison to grow billable caregiving hours by building and maintaining relationships with hospital discharge planners, physicians, skilled nursing facilities and community organizations. Qualifications

• Bachelor’s degree in business, marketing or communications preferred, or equivalent experience.
• Two years in sales/marketing and referral development; healthcare sales desirable.
• Excellent sales, communication and customer service skills.
• Positive and joyful attitude.
• Good computer skills.
• Reliable transportation, valid driver’s license and auto insurance.

Responsibilities

• Visit referral sources in person weekly to build relationships and referrals.
• Develop partnerships with hospitals, physician practices, senior living communities, Home Healths, Hospices.
• Cultivate legal and financial referrals (estate/trust attorneys, fiduciaries, wealth managers).
• Cultivate community referrals (churches, Chambers of Commerce, Alzheimer’s/ALS/Parkinsons groups).
• Represent the agency at community events to raise brand awareness.
• Provide training to referral sources on value of private duty care.
• Track referral source contacts and outcomes in the referral/visit log.
• Collaborate with care coordinator to convert referrals into billable hours.
• Report activity and pipeline status weekly.
