Job summary

Assisting Hands Home Care in Florida seeks a Community Liaison to expand its referral network and grow the client base. This relationship-driven sales role covers Polk and Highlands counties, meeting with discharge planners, care managers, social workers, and elder care attorneys to convert referrals into new clients.

You are self‑motivated, accountable to your numbers, and comfortable representing the agency at community events.

Qualifications

• 3+ years in sales, business development, or referral marketing.
• Valid Florida driver’s license and a reliable vehicle.
• Strong verbal and written communication.
• Comfortable using a CRM and basic office software.
• Ability to manage own schedule and meet targets.

Responsibilities

• Build and grow referral relationships with hospital discharge planners, case managers, care managers, social workers, facilities, elder care attorneys.
• Get in front of new and existing referral sources weekly and turn visits into client referrals.
• Log contact and visit information in our CRM.
• Meet with prospective clients and families to explain services and start care.
• Represent Assisting Hands at industry events, health fairs, and community events.
• Coordinate with the office team on intakes and hand-offs for smooth onboarding.
