COMPANION AT HOME COMMUNITY LIAISON/MARKETER
JOB TITLE: Community Liaison
REPORTS TO: Assistant Administrator
SUMMARY: The Community Liaison is a field-based role responsible for growing billable caregiving hours by building and maintaining relationships with referral sources — hospital discharge planners, physicians, skilled nursing and assisted living facilities, case managers, and community organizations. This person spends the majority of their week in person, not behind a desk, generating a steady, reliable pipeline of new client admissions and increased billable hours.

JOB-SPECIFIC DUTIES AND RESPONSIBILITIES:
- Conduct in-person visits to referral sources on a weekly, recurring basis, building relationships that translate into consistent referral flow.
- Identify and cultivate new referral partnerships with healthcare related referral sources like; hospitals, physician practices, senior living communities, Home Healths, Hospices, etc.
- Identify and cultivate new referral partnerships with legal and finance referral sources like; estate & trust attorneys, long term care fiduciaries, wealth managers and financial planners.
- Identify and cultivate new referral partnerships with community referral sources like; churches, Chambers of Commerce, Alzheimers Association, ALS Associations, and Parkinsons Associations.
- Represent the agency at community events, health fairs, and lunch-and-learns to build brand awareness.
- Provide training and education to referral sources on the general value of private duty care and the specific value our agency brings to customers and referral sources.
- Track all referral source contacts, visit cadence, and outcomes in the agency's referral/visit log.
- Partner with care coordinator & scheduler to ensure referrals convert quickly and reliably into billable hours, protecting the agency's reputation with referral sources.
- Report on visit activity, new leads, and pipeline status to both one-on-one with supervisor and in weekly team meetings.
RELATED DUTIES:
- Attends and participates in one-on-one meetings, weekly team meetings and monthly staff meetings.
- Within departmental parameters, exhibits excellent customer service, decision-making and problem-solving skills.
- Effectively prioritizes and coordinates job tasks for maximum productivity.
- Perform additional tasks as assigned by supervisor or Administrator.
MINIMUM REQUIREMENTS:
- Bachelor’s degree in business, marketing or communications preferred, or a combination of education and experience.
- Two years experience in sales/marketing and referral development; health care facility or medical related sales experience highly desirable.
- Demonstrated superior sales, communication and customer service skills.
- Positive & joyful attitude.
- Good computer skills.
- Reliable transportation, current driver license and current auto insurance.
SALARY LEVEL: Exempt
