MC Community Services is seeking an experienced case manager to join our Section 17 adult services team. This role involves working in the community with adults living with serious mental illness, performing assessments, coordinating services, and ensuring supports are effective and in place.
You will conduct biopsychosocial assessments, connect clients to community resources, build trusting relationships, coordinate care across providers, monitor outcomes, and maintain thorough documentation.

We\'ve just been approved to provide Community Integration Services across Penobscot and Piscataquis counties in Maine, and we need someone who knows how to connect people with the supports that keep them stable, independent, and engaged in their community.
This is not a desk job. You\'ll be out in the community working directly with adults living with serious and persistent mental illness — conducting assessments, building relationships, coordinating services, and making sure the supports around each person are actually working. You\'ll be the person who knows each client\'s full picture and makes sure nothing falls through the cracks. This is W-2 employment, with real supervision and a team behind you.

Active MHRT/C (Mental Health Rehabilitation Technician — Community) certification required. Experience in case management, community integration, or similar community-based behavioral health work. Strong organizational and documentation skills. Valid Maine driver\'s license and reliable transportation. Knowledge of Penobscot/Piscataquis community resources a strong plus.
