Qualifications

• At least 1 year of health care experience with underserved populations or equivalent.
• CHW certification may be required for certain states depending on contract requirements.
• Valid unrestricted driver's license, reliable transportation and adequate auto insurance for travel.
• Ability to multitask while speaking with members.
• Excellent customer service, organizational and time-management skills.
• Willingness to learn other programs and software systems.
• Excellent verbal and written communication skills.
• Proficiency with Microsoft Office applications.

Responsibilities

• Engages with members as an advocate and resource to support health care needs.
• Collaborates with health care services team providing non-clinical paraprofessional duties in the field (in homes, shelters, provider offices, etc.).
• Helps members navigate and maximize health plan benefits (scheduling, transportation, prescriptions, follow-ups).
• Assists members in accessing social services (housing, food, employment).
• Performs outreach to reconnect disconnected members with special needs.
• Performs research to locate hard-to-reach members using internal data and community resources.
• Participates in outreach activities (telephonic and face-to-face).
• Guides members to maintain Medicaid eligibility and explore financial resources.
