IMPaCT Care in Los Angeles, CA is seeking a Community Health Program Coordinator to act as the operational backbone for CHW teams across multiple sites. You will coordinate program activities, manage data collection and analysis, and ensure smooth program operations.

The role emphasizes collaboration, meticulous documentation, and the ability to engage diverse communities. Travel to multiple locations may be required; no remote work.

Qualifications

• 3–5 years in program coordination, research administration, or equivalent experience.
• Strong writing and communication skills.
• Proven experience with data collection, analysis, and protocol implementation.
• Proficient in Microsoft Word and Excel.
• Valid driver's license with car insurance.

Responsibilities

• Support 1-2 Program Managers overseeing CHW teams serving hundreds of patients.
• Schedule training sessions, team meetings, and community events.
• Maintain program documentation, client records, and compliance requirements.
• Collect, organize, and analyze data to measure outcomes and improve program quality.
• Engage with community members, colleagues, and partners, including those who may be upset.
• Enroll patients into the program.

• Ability to travel to multiple locations across the Atlanta area
• Bachelor's degree preferred
• Ability to pass background check and pre-employment screening
