Job summary

Eminence Home Care is seeking a Community Liaison - Business Development to grow our referral network across Pennsylvania. This field-based role focuses on identifying new referral opportunities, maintaining partnerships, and educating healthcare professionals and families about our non-medical home care services.

The ideal candidate is outgoing, self-motivated, organized, and passionate about helping others.

Qualifications

• Experience in business development or outside sales.
• Ability to travel regularly throughout Pennsylvania territory.
• Valid driver's license and reliable transportation.
• Proven ability to develop professional relationships and achieve measurable goals.
• Excellent interpersonal and presentation skills.

Responsibilities

• Identify and develop relationships with new referral sources, including hospitals, facilities, physician offices, and community organizations.
• Promote Eminence Home Care's non-medical services to healthcare professionals, potential clients, and families.
• Conduct regular in-person visits, presentations, and informational meetings to educate referral sources.
• Represent Eminence Home Care at health fairs, senior events, and community outreach initiatives.
• Maintain consistent communication with referral partners and ensure smooth transitions from referral to start of care.
• Maintain a strategic weekly outreach plan and track activities in the CRM.
