Job summary

Eminence Home Care is seeking a motivated Community Liaison to grow our referral network across Pennsylvania. You will win new referral sources, deepen existing relationships, and plan your time strategically to ensure every visit has a purpose.

You will own a defined territory and be measured on referrals, starts of care, and account growth. Ideal candidates are relationship-driven, comfortable meeting hospital and rehab staff, and skilled at planful, proactive outreach.

Qualifications

• Experience in business development, sales, or account management with a track record of meeting measurable goals.
• Strong interpersonal and communication skills; able to build trust quickly.
• Highly organized and self-motivated; can plan your own week and manage multiple accounts.
• Comfortable with field-based work and travel across PA counties.
• Valid driver's license and reliable transportation.

Responsibilities

• Drive new business by identifying and pursuing referral sources (hospitals, rehab facilities, physicians, case managers, social workers, community orgs).
• Turn introductions into referrals by understanding partner needs and offering solutions.
• Re-engage at-risk accounts and involve management early to protect opportunities.
• Maintain regular contact with partners; deepen relationships with multiple contacts at each account.
