R&C Consulting Group, Inc. seeks a CST Team Lead to guide a field-based, multidisciplinary team delivering community-based, recovery-oriented care for adults with serious mental illness and co-occurring needs.

You will supervise QPs/APs/PPs/Peer Support, coordinate assessments and crisis plans, and lead weekly team reviews while participating in a 24/7 on-call rotation. You will ensure documentation and build partnerships with PCPs and community providers.

Qualifikationen

• Master’s in a human-services field.
• Qualified Professional (QP) status.
• Full or provisional clinical license (LCMHC/LCMSW/LMFT/LCAS).
• 1+ year post-graduate experience with adult mental health.
• Current First Aid/CPR/BBP and crisis-intervention training.

Aufgaben

• Provide clinical leadership for a CST serving adults with SMI/SUD; services are home & community-based.
• Coordinate assessments and crisis plans with measurable objectives.
• Run weekly team case reviews and assign staff by acuity.
• Participate in 24/7/365 on-call rotation with timely post-crisis follow-up.
• Deliver direct interventions within your license/scope and coordinate psychotherapy when needed.
• Ensure clean, timely documentation meeting medical-necessity and records standards.
• Build partnerships with PCPs, hospitals, courts/probation, housing/employment supports, and community providers.
