Responsibilities

• Provide integrated whole-person care management under the CFSP Care Management model.
• Coordinate across physical health, behavioral health, I/DD, LTSS, pharmacy, and unmet health needs.
• Collaborate with County Child Welfare Workers to align healthcare with permanency goals.
• Identify members with health care needs and coordinate with the care team for appropriate interventions.
