This is a community-based position—not a traditional remote job.
Administrative work may be completed from a home-based workspace, but regular in-person community work is an essential part of the position.
You will routinely travel throughout your assigned service region.
Meetings may occur in individuals' homes, provider locations, workplaces, programs, and other community settings.
Travel varies according to caseload needs and may occasionally involve driving up to approximately two hours one way.

As a CCS, you will develop relationships with individuals with intellectual and developmental disabilities and their families and coordinate services around their needs, preferences, choices, and goals.
Responsibilities include:
Case management does not end when the meeting or phone call is over. CCS employees are responsible for documenting the services they provide accurately and within required timelines. Documentation must clearly reflect the activity performed, the person's needs or goals, the connection between activity and the person's goals, and appropriate next steps. This position also includes defined productivity expectations.
Success requires the ability to balance:
Travel between work locations is part of the workday but is not itself a billable case management service. Thoughtful scheduling and caseload management are therefore important to success in the role.
CCS employees spend significant portions of their workday independently managing their caseloads, schedules, documentation, and community visits. At the same time, case management at Optimal is a team effort. You will work closely with supervisors, other Case Managers, Quality Assurance, Training, providers, families, community partners, and organizational leadership.

Regular travel throughout the assigned counties and service region is required.
Community work may occur in homes, workplaces, programs, provider locations, and other public or private settings.
Work environments may occasionally be unfamiliar, uncomfortable, or present potential health or safety concerns.
Appropriate personal protective equipment may be required depending on the environment.
Employees are expected to exercise professionalism, situational awareness, sound judgment, and appropriate safety practices while working independently in the community.
