Job summary

Bluestone Physician Services is seeking a Geriatric Complex Care Manager to support complex, chronically ill patients in senior living communities. This field-based role acts as the primary link among patients, families and primary care providers to execute unified care plans and reduce unnecessary hospitalizations.

The position is 90% field-based with a daytime schedule, offering meaningful impact in Minnesota communities served by Bluestone, including Edina and Richfield.

Qualifications

• Bachelor's degree or higher in a relevant field.
• 3–5 years in value-based care, population health, case management or discharge planning.
• Behavioral health experience preferred.

Responsibilities

• Develop and manage individualized, comprehensive care plans.
• Coordinate behavioral health interventions within the CoCM model.
• Collaborate with MDs, CNPs and PAs for real-time care updates.
• Close gaps in care and optimize quality measures and utilization.
