The Buckeye Ranch is hiring a Care Management Coordinator to support OhioRISE Care Management Entity in Franklin County and surrounding areas. This role involves working with children, youth, caregivers, providers, and agencies to coordinate family-centered, trauma-informed care.

You will travel regionally, document visits, and develop individualized care plans while ensuring timely communication and collaboration across multi-system partners.

Qualifications

• Direct experience with children or youth in behavioral health or related services.
• Three years of relevant work with a HS diploma/associate; two years with BA; or a master's.
• Ability to engage families and coordinate multiple providers.
• Active driver's license and reliable transportation.

Responsabilités

• Build trusting relationships with children, youth, and caregivers.
• Assess needs using CANS and HF Wraparound; develop and update care plans.
• Coordinate services among families, providers, schools, child welfare, and community resources.
• Maintain family contact, document visits, and follow referrals and tasks.
