Buckeye Ranch in Whitehall, OH is seeking a Care Management Coordinator to help families navigate complex behavioral health needs within the OhioRISE framework. This community-based role partners with children, youth, caregivers, providers, and community agencies to coordinate family-centered, trauma-informed care.

• Three years of relevant experience with a high school diploma or associate degree; two years with a bachelor’s degree in a human-services field; or a master’s degree in a human-services field.
• Active driver’s license and reliable transportation are required.

Responsabilidades

• Build trusting relationships with children, youth, and caregivers to involve families in decision-making.
• Assess behavioral health and practical needs using appropriate tools and develop care plans.
• Coordinate services among families, providers, schools, child welfare, and community resources.
• Document visits and referrals, complete related tasks, and ensure timely follow-up.
• Be available for two evenings per week for client and family visits and participate in on-call rotation.
