The Buckeye Ranch is hiring a Care Management Coordinator for its OhioRISE Care Management Entity. This community-based role partners with children, youth, caregivers, providers, and schools to coordinate family-centered, trauma-informed care.

You will assess needs using CANS and High Fidelity Wraparound, develop individualized care plans, and coordinate services across multiple systems. Strong documentation and relationship-building are essential.

Formación

• Direct work with children or youth in behavioral health, child welfare, developmental disabilities, juvenile justice, or related human-services setting.
• Three years of relevant experience with a high school diploma or two years with a bachelor’s degree (or master’s) in a human-services field.
• Active driver’s license, reliable transportation, and proof of auto insurance.
• Ability to engage families with complex needs, coordinate multiple providers, manage priorities, and document clearly and timely.

Responsabilidades

• Build trusting relationships with children, youth, and caregivers and keep families involved in decisions about care.
• Assess behavioral health and practical needs using CANS and High Fidelity Wraparound; develop and update individualized care plans.
• Coordinate services and lead collaboration among families, behavioral health providers, schools, child welfare, and community resources.
• Maintain required family contact, complete timely documentation, and follow through on referrals and care-plan tasks.
• Travel to family homes, schools, and community settings as needed.
