Job summary

CVS Health Corporation is seeking a full-time Field-Based Care Coordinator in Ohio to support the OhioRISE program. The role requires travel to member homes within the Northeast Ohio region, with frequent after-hours work to meet member needs.

The position emphasizes care coordination, education for members, and cross-team collaboration to ensure high-quality, equitable behavioral health services for children and families in a multi-agency environment.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with personal computers, keyboard and MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, developmental disabilities, juvenile justice, or public sector human services.
• 2+ years experience in one or more areas: family systems, case management, child counseling/therapy, etc.
• 2+ years experience with Ohio delivery systems and local networks.
• Willing and able to travel within the assigned region 50%+ of the time; some travel to New Albany may be required.
• Able to work beyond core hours as needed.

Responsibilities

• Coordinate and implement care plan activities and monitor progress.
• Evaluate members' needs/eligibility using care management tools.
• Collaborate with case managers, Medical Directors, and other programs to overcome barriers.
• Serve as single point of contact for members and support access to services.
• Utilize quality management processes and regulatory guidelines in care delivery.
