Job summary

Conduent is seeking a Field-Based Medical Benefit Review Services Associate to support Louisiana Medicaid by conducting in-home assessments and coordinating services for older adults and individuals with disabilities. You will travel within Assumption, Lafourche, St. Charles, St. James, St.

John the Baptist, St. Mary, and Terrebonne Parishes. The role includes training in Louisiana Medicaid and HCBS programs, with opportunities to obtain IHC Certification and gain experience in case management

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Public Health, Healthcare Administration, or related field.
• Strong verbal and written communication skills.
• Interest in working directly with diverse populations.
• Louisiana resident with reliable transportation.
• Valid driver's license, current auto insurance, and ability to travel daily.

Responsibilities

• Conduct in-home assessments for LT-PCS applicants and participants.
• Evaluate client needs for eligibility determinations, care planning, and service coordination.
• Provide Choice Counseling for HCBS programs when requested.
• Educate applicants and families about Louisiana Medicaid long-term care programs and resources.
• Develop and maintain person-centered Plans of Care based on needs and assessments.
• Coordinate services and connect clients with healthcare providers and community resources.
• Complete monitoring visits, client surveys, case documentation, and quality management activities.
• Maintain accurate records per state and program guidelines.
