Job summary

Conduent is seeking a field-based Medical Benefit Review Services Associate to support Louisiana Medicaid and long-term care programs. You will conduct in-home assessments, coordinate services, and help older adults and individuals with disabilities access healthcare and community resources to stay safely in their homes.

This role requires regular travel within Avoyelles, Rapides, and Vernon Parishes, with pay rate of $19.50 per hour and remote work options as needed.

Qualifications

• Bachelor's degree required in a related field.
• Strong verbal and written communication skills.
• Must be a Louisiana resident with reliable transportation and ability to travel daily.

Responsibilities

• Conduct in-home assessments for LT-PCS participants.
• Evaluate client needs for eligibility determinations, care planning, and service coordination.
• Provide Choice Counseling for HCBS programs when requested.
• Educate applicants and families about Medicaid long-term care programs and resources.
• Develop person-centered Plans of Care based on needs and assessments.
• Coordinate services with healthcare providers and community organizations.
• Complete monitoring visits, client surveys, and documentation.
