Job summary

Community Bridges, Inc. (CBI) is seeking a Rehabilitation Specialist to support SMI clients in daily functioning and community integration. You will collaborate with the Independent Living, Housing and Employment Specialists, Case Manager and Clinical Coordinator to arrange needed services and monitor progress.

The role requires mobility with 80% field time, driving to client sites and coordinating services across multiple settings to ensure comprehensive care.

Qualifications

• High school diploma or GED required; associate degree or higher preferred.
• 1 year experience with the SMI population; experience in multiple settings preferred.
• 1 year experience in a behavioral health role providing outreach and engagement activities preferred.
• Minimum six months recovery from substance use and/or mental health disorders preferred.
• Valid AZ Driver’s License in good standing.

Responsibilities

• Assist SMI clients in obtaining or maintaining needed supports for daily functioning.
• Coordinate with Case Manager and Clinical Coordinator to ensure services are delivered.
• Spend approximately 80% of time in the field reviewing progress and arranging additional services.
