Job summary

CVS Health is seeking a field-based behavioral health clinician in Kentucky to apply advanced clinical judgment, assess member needs, coordinate care, and collaborate with diverse providers across the health system. You will conduct psychosocial assessments, develop care plans, support transitions of care, and connect members to community resources.

Qualifications

• Masters degree in Social Work, Counseling, Psychology, Human Services or a related field.
• Active, unrestricted licensure as a behavioral health professional in accordance with state requirements.
• Minimum of three (3) years of experience in behavioral health, case management, care coordination, or related setting.

Responsibilities

• Conduct behavioral health and psychosocial assessments.
• Develop, implement, and monitor individualized care plans.
• Coordinate services across behavioral health, medical, and community providers.
• Support discharge planning and transitions of care.
• Advocate for members and connect them to community resources.
• Monitor member progress, identify barriers, and facilitate problem-solving to improve outcomes.
• Participate in interdisciplinary care team meetings and provider collaboration.
