Job summary

CVS Health is seeking a field-based behavioral health professional to assess, plan, and coordinate care across providers in the Eastern Mountain Region of Kentucky. The role involves extensive collaboration with medical and community teams to optimize outcomes for members.

Requirements include a master's degree in a related field and active licensure (LCSW/LPC/LMFT/psychologist) with at least three years of relevant experience. Travel and flexible scheduling are required.

Qualifications

• Masters degree in Social Work, Counseling, Psychology, Human Services, or a related field
• Active, unrestricted licensure as an LCSW, LPC, LMFT, Psychologist, or equivalent in the state
• Minimum of three years of behavioral health, case management, or care coordination experience
• Experience with behavioral health assessments and member-centered care plans
• Knowledge of mental health, substance use disorders, trauma-informed care and community services

Responsibilities

• Conduct behavioral health and psychosocial assessments.
• Develop, implement, and monitor individualized care plans.
• Coordinate services across behavioral health, medical, and community providers.
• Support discharge planning and transitions of care.
• Advocate for members and connect them to community resources.
• Monitor member progress and facilitate problem-solving to improve outcomes.
• Participate in interdisciplinary care team meetings and provider collaboration.
