CVS Health is seeking a field-based Behavioral Health Specialist in Kentucky to apply advanced clinical judgement, coordinate care, and direct provider collaboration. The role emphasizes psychosocial wraparound services, cost-effective outcomes, and leveraging community resources.

Travel across the Eastern Mountain Region is required. Requires a masters in a related field, active licensure, and at least 3 years of behavioral health experience.

Qualifications

• Masters degree in Social Work or related field is required.
• Active, unrestricted licensure in accordance with state requirements.
• Minimum three years of behavioral health, case management, or care coordination experience.
• Experience assessing behavioral health and developing member-centered care plans.
• Knowledge of mental health, substance use disorders, trauma-informed care, and community services.

Responsabilités

• Conduct behavioral health and psychosocial assessments.
• Develop, implement, and monitor individualized care plans.
• Coordinate services across behavioral health, medical, and community providers.
• Support discharge planning and transitions of care.
• Advocate for members and connect them to community resources.
• Monitor progress, identify barriers, and facilitate problem-solving.
• Participate in interdisciplinary care team meetings and provider collaboration.
