Job summary

CVS Health is seeking a licensed behavioral health professional to conduct behavioral health assessments, develop and monitor care plans, and coordinate services across providers within the Eastern Mountain Region of Kentucky. The role involves 50% field travel and flexible scheduling to support discharge planning and member transitions.

Ideal candidates hold licensure (LCSW/LPC/LMFT/psychologist) with a Master’s in a relevant field and at least three years of behavioral health experience.

Qualifications

• Active, unrestricted licensure in specified behavioral health professions in accordance with state requirements.
• Masters degree in Social Work, Counseling, Psychology, Human Services, or related field.
• Minimum of three years of behavioral health, case management, or care coordination experience.
• Experience conducting behavioral health assessments and developing member-centered care plans.
• Knowledge of mental health, SUD, trauma-informed care, recovery principles, and community services.
• Experience coordinating care across providers, families, and community systems.
• Strong communication, clinical judgment, documentation, and organizational skills.

Responsibilities

• Conduct behavioral health and psychosocial assessments.
• Develop, implement, and monitor individualized care plans.
• Coordinate services across behavioral health, medical, and community providers.
• Support discharge planning and transitions of care.
• Advocate for members and connect them to community resources.
• Monitor member progress and identify barriers to improve outcomes.
• Participate in interdisciplinary care team meetings and provider collaboration.
