Job summary

CVS Health Corporation is seeking a Case Management Coordinator to support Ohio Medicaid members through comprehensive care coordination and plan execution. This full-time, field-based telework role requires strong collaboration with care teams and the ability to travel within Northeast Ohio 50% of the time or more.

Ideal candidates have 2+ years in behavioral health or related fields, proficiency with MS Office, and a commitment to delivering high-quality, patient-centered care.

Qualifications

• 2+ years in behavioral health, social services, or human services.
• 2+ years using MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• 2+ years in children’s mental health, child welfare, or related fields; community-based services to children/youth and families.

Responsibilities

• Coordinate and implement care plan activities and monitor progress.
• Collaborate with case managers, supervisors, Medical Directors, and health programs to overcome barriers.
• Engage with Child and Family Teams and escalate quality of care issues as needed.
• Provide coaching and support to empower members to make informed health decisions.
• Serve as a single point of contact for members and remediate care gaps.
