Job summary

OptumCare seeks a Behavioral Health Advocate to support enrollees in the Buffalo area. You will perform case management and care coordination for adult Medicaid members, meeting them at hospitals, clinics, community sites, and in their homes.

The role is field-based with 80% in-person work and 20% remote, Monday–Friday 8am–5pm, no weekends. A NY license and relevant degree are required, with residency near Buffalo.

Qualifications

• Master's or Doctoral degree in Psychology, Social Work, Counseling, Marriage and Family Therapy, or a related field.
• Active, unrestricted NY license: LMHC, LMFT, LCSW, or LP.
• 2+ years of experience in a related mental health environment.
• Proficient in Word, Outlook, Excel, and Teams.

Responsibilities

• Identify and provide community service linkage.
• Assist enrollees with aftercare appointments.
• Provide follow-up as determined by member needs.
• Identify gaps in care and problem-solve for access to services.
• Review census to identify re-admissions within 12 months.
• Go to facility meetings to develop aftercare plans.
• Manage aftercare follow-up for high-risk enrollees.
