Optum is seeking a Behavioral Health Advocate to support Medicaid enrollees in Suffolk County, NY. The role combines field-based case management with remote work, focusing on linkage to community services and aftercare planning to prevent hospitalizations.

The position requires licensure in NY, a relevant advanced degree, and 2+ years in mental health, with travel across the assigned territory and a Monday–Friday schedule.

Formación

• Master's or Doctoral degree in Psychology, Social Work, Counseling, Marriage and Family Therapy, or a related field.
• Active, unrestricted NY license as LMHC, LMFT, LCSW or LP.
• 2+ years of experience in a related mental health environment.
• Proficient in Microsoft Office including Word, Outlook, Excel, and Teams.

Responsabilidades

• Identify and provide community service linkage.
• Assist enrollees with aftercare appointments.
• Provide subsequent member follow-up as determined by individual needs.
• Work with enrollees to identify gaps in care or obstacles to care and problem solve for successful connection to needed services.
• Review IP Census daily to determine re-admits within the last 12 months.
• Go to facility meetings with treatment team to develop an aftercare plan.
• Manage aftercare follow-up of identified high risk enrollees (calls/outreach and documentation).
• Meet with community providers to foster rapport and identify new programs.
