Optum, a global health services leader, seeks a Behavioral Health Advocate in Suffolk County, NY. This field-based role coordinates case management for adult Medicaid members, meeting them at hospitals, clinics, or in their homes to support treatment goals.

The schedule is Monday–Friday, 8am–5pm with no weekends. 80% in-person, 20% remote; relocation near Riverhead preferred. You will liaise with care teams, address gaps, and connect members with community resources to prevent hospitalization.

Formación

• Master's or Doctoral degree in Psychology, Social Work, Counseling, Marriage and Family Therapy, or a related field.
• Active, unrestricted NY license (LMHC/LMFT/LCSW/LP).
• 2+ years in a related mental health environment.
• Proficient in Microsoft Office (Word, Outlook, Excel, Teams).
• Dedicated home office with secure high-speed internet.
• Valid driver's license and daily travel across territory.

Responsabilidades

• Identify and provide community service linkage.
• Assist enrollees with aftercare appointments and follow-ups.
• Collaborate with treatment teams to develop aftercare plans.
• Engage enrollees to solve gaps in care and access services.
• Attend facility meetings to ensure smooth aftercare transitions.
