Job summary

OptumCare is seeking a Behavioral Health Advocate in the Buffalo, NY area. The position involves field-based case management and care coordination for adult Medicaid members, with 80% in-person visits and 20% remote work. The schedule is Monday–Friday, 8am–5pm, with residency in the Buffalo area required.

The role requires a Master's or Doctoral degree in a relevant field and NY licensure (LMHC/LMFT/LCSW/LP), plus 2+ years of related experience. Travel across the territory is expected.

Qualifications

• Master's or Doctoral degree in Psychology, Social Work, Counseling, Marriage and Family Therapy, or a related field.
• Active, unrestricted, independent license in NY: LMHC, LMFT, LCSW, or LP.
• 2+ years of experience in a related mental health environment.
• Proficient in Microsoft Office including Word, Outlook, Excel, and Teams.
• Dedicated home office with secure high-speed internet at home.
• Valid driver's license and reliable transportation for daily travel in Buffalo area.
• Reside in or near Buffalo, NY.

Responsibilities

• Identify and provide community service linkage.
• Assist enrollees with aftercare appointments.
• Provide follow-up as determined by member needs.
• Identify gaps in care and problem-solve for connections to services.
• Review IP Census daily for re-admits within last 12 months.
• Attend facility meetings to develop aftercare plans.
• Manage aftercare follow-up for high-risk enrollees.
• Meet with providers/facilities to foster rapport and identify new programs.

Education

Master's or Doctoral degree in Psychology, Social Work, Counseling, Marriage and Family Therapy, or a related field

Active NY license: LMHC, LMFT, LCSW, or LP
