Job summary

Optum is seeking a Behavioral Health Advocate to coordinate care for adult Medicaid members in and around Buffalo, NY. You will travel to hospitals, clinics, and homes, with 80% fieldwork and 20% remote work from home. Residency in the Buffalo area is required, with a 8am-5pm weekday schedule.

Responsibilities include linking to community services, arranging aftercare, and collaborating with providers to ensure continuous care and prevent hospitalizations.

Qualifications

• Master's or Doctoral degree in Psychology, Social Work, Counseling, Marriage and Family Therapy, or a related field.
• Active, unrestricted NY license as LMHC/LMFT/LCSW/LP.
• 2+ years of experience in a related mental health environment.
• Proficient in Microsoft Office including Word, Outlook, Excel, and Teams.

Responsibilities

• Identify and provide community service linkage.
• Assist enrollees with aftercare appointments.
• Provide subsequent member follow-up as determined by individual member needs.
• Work with enrollees to identify gaps in care and problem solve for connection to needed services.
• Review IP Census daily to identify re-admits within the last 12 months.
• Attend facility meetings with treatment team to develop an aftercare plan.
• Manage aftercare follow-up of high risk enrollees.
• Meet with community providers to foster rapport and identify new programs.
