Job summary

NorthCare seeks a Care Navigator Level 2 to join the ACT team, delivering case management, rehab, and wellness support directly in the community. You will meet clients where they are, provide medication prompts, and coordinate with partners to improve outcomes.

The role requires CM II certification, wellness coaching credentials, and at least two years' CMII experience in community mental health, with travel across service areas.

Qualifications

• CM II certification required for case management work in mental health.
• Wellness coach certification or willingness to obtain it.
• Minimum two years CMII experience in community mental health.
• Valid Oklahoma driver’s license with reliable transportation.
• Ability to travel extensively in community settings.

Responsibilities

• Provide integrated case management, rehab, medication prompts in community.
• Coordinate care across physical/behavioral health with outside providers.
• Assist housing, employment, education, transportation access for clients.
• Promote self-management and health education; advocate for clients.
• Provide crisis support, safety planning, hospitalization coordination.
• Mentor peers and lead care coordination meetings for complex cases.
• Transport clients to resources and appointments; document services.
• Meet productivity standards and complete required trainings.
