Job summary

Plymouth Housing is seeking a Foundational Community Supports Specialist to strengthen Medicaid-funded services and ensure policy translated into practice. You’ll help staff document accurately and stay compliant with billing requirements, while supporting program integrity.

The role blends training, coaching, and data management to ensure FCS services are delivered consistently and align with Medicaid standards.

Qualifications

• 1+ year experience with FCS or Medicaid-funded services.
• 2+ years writing and reviewing clinical or related documentation.
• 2+ years providing training, facilitation, or coaching.

Responsibilities

• Deliver onboarding and ongoing training for staff on FCS rules, documentation, and billing.
• Translate Medicaid and FCS requirements into clear tools and guidance.
• Provide on-site coaching and technical support to improve workflow.
• Monitor and maintain accurate FCS enrollment and service data across systems.
• Collaborate with internal partners to evaluate documentation trends and ensure compliance.
