Job summary

The University of North Carolina at Chapel Hill is seeking a part-time Human Services Practitioner-CIND in North Carolina. This hybrid role supports families of children with IDD by providing navigation, education, and linkage to community resources through UNC pediatrics and partner agencies.

The position offers approximately 20 hours per week, with some tasks completed remotely and others on-site in pediatric settings.

Qualifications

• Associate’s degree in a related Human Services discipline and two years of experience.
• All degrees must be received from appropriately accredited institutions.
• Experience in patient/family navigation preferred.

Responsibilities

• Serve as a family system navigator to support I/DD families and connect them to local resources.
• Educate, train and guide practice staff on effectively supporting children with IDD and families.
• Collaborate with UNC pediatric providers, community partners and managed care organizations.
• Document referrals and outcomes; participate in weekly team meetings and ensure HIPAA compliance.
