Job summary

Children’s Services of Roxbury seeks a Family Partner to support parents and caregivers of youth under 21, providing strength-based guidance and resources. This full-time role at the Roxbury site requires 40 hours weekly and travel to multiple locations.

The Family Partner will link families to community resources, participate in all care planning, and collaborate with care teams to improve outcomes for youth and families across Massachusetts.

Qualifications

• Experience as a caregiver of a youth with special needs.
• Experience navigating child and family–serving systems.
• Excellent writing and communication skills.
• Must have a current valid driver’s license and reliable transportation.
• Bachelor’s degree in human services or an Associate’s degree with relevant experience, or high school diploma with two years’ experience.
• Provide culturally competent services respecting diversity.

Responsibilities

• Engage the parent/caregiver with learning how to network to community resources and services.
• Work with family and behavioral health providers to implement treatment plan objectives.
• Participate in all care planning meetings for the youth.
• Interact with agencies to support care planning and coordination.
• Coordinate and attend Care Planning Team and treatment team meetings.
• Attend weekly CSA staff meetings.
• Report weekly to Senior Parent Partner and supervisor.
• Complete all paperwork and data entry timely.
