Job summary

Children’s Services of Roxbury is seeking a Family Partner to provide family support and training for youth under 21 in homes and community settings. The role emphasizes strengthening caregiver capacity to improve youth functioning as outlined in treatment plans.

The position is full-time at the Roxbury site with travel to multiple locations. Ideal candidates will have caregiving experience with youth, strong writing skills, and a driver’s license.

Qualifications

• Experience caring for a youth with special needs or mental health needs.
• Strong writing and communication abilities required.
• Ability to navigate child/family serving systems and coordinate care.
• Current valid driver's license and reliable transportation.

Responsibilities

• Engage the parent/caregiver to link to community resources and services.
• Collaborate with families and care providers to implement treatment goals.
• Participate in care planning meetings for the youth.
• Coordinate with various agencies to support services and care planning.
• Attend Care Planning Team meetings and weekly staff meetings.
• Provide timely documentation and data entry for cases.
