Qualifications

• Bachelor’s degree in Social Work, Human Services, or a related field, or 5+ years of direct case management experience.
• At least 1 year of experience working with families or children with diverse needs.
• Strong verbal and written communication skills.

Responsibilities

• Conduct 1-2 weekly home visits to provide support and coaching.
• Help families navigate various resources including education and housing.
• Collaborate with DCF and other providers weekly.
