Youth Villages is seeking a community-based professional to support youth and families in Memphis through case management, crisis intervention, and therapeutic in-home services. The role emphasizes collaboration, flexibility, and thorough documentation in EMR to ensure safety and progress.

Applicants should have a Bachelor's degree, be familiar with local resources, and demonstrate strong organizational and communication skills across multiple teams and settings.

Qualificações

• Experience with case management preferred.
• Experience working with youth and families in crisis preferred.
• Strong organizational skills and attention to detail.
• Excellent written, verbal, and oral skills.

Responsabilidades

• Carries a caseload of approximately 30 youth.
• Conducts weekly in-person sessions as needed.
• Provides therapeutic services in the home and community locations.
• Links youth and families to resources as needed.
• Collaborates with partners and services involved with youth and families.
• Maintains timely, accurate documentation in EMR.
